Well Visit Planner: Helpful Tool For Families of Children Ages Birth to Age 6

The Well Visit Planner (WVP) is a tool that parents or other people raising children ages birth to 6 can use to track their children’s development and prepare for their children’s medical appointments. The Child and Adolescent Health Measurement Initiative (CAHMI) designed the WVP based on Bright Futures Guidelines for Health Supervision of Infants, Children and Adolescents, Fourth Edition, which most medical providers who see young children use.

The WVP supports families in planning for their children’s medical visits. It helps them decide what they want to discuss and their questions. It also provides valuable resources and information about different topics. The planner is available for the First Week, 1, 2, 4, 6, 9, 12, 15, 18-month and 2, 2.5, 3, 4, 5 and 6-year well visits.

One of the most helpful things about the planner is that families are entirely in charge of the information. You can complete the planner on your phone or computer and get feedback about your child’s development. You can then decide if you want to share the information with your child’s medical provider or anyone else working with your child.

Families as Allies thinks the Well Visit Planner could be a helpful tool for families, but we want to know what families of young children think. We are asking families of children ages birth to 6 to complete the Well Visit Planner (your answers are confidential, and no one will see them unless you share them) and then let us know what you think about the planner.
